Job Description:
A company is looking for an Analyst, Case Management
Specialist.
Key Responsibilities
Interact with low stratification members via phone, coordinating
care and updating assessments and care plans
Create care plans based on assessments and claims data, and
participate in Interdisciplinary Team meetings
Identify and link members with health plan benefits and community
resources while performing administrative tasks
Required Qualifications
3 plus years of experience in a health-related field
2 plus years of customer service experience
2 plus years of experience with claims
Bachelor's degree or equivalent work experience required
Master's degree and relevant certifications preferred
